This is a PDF of my hand made templates. I did my best to digitize them and scale them to size. Each file has a ruler on page 1 to double check scale.

I highly recommend printing one page first to check scale. (Windows is weird, so I suggest printing from Adobe Acrobat to ensure proper printing.)
